Eligibility criteria




To be eligible for QPP.AZ, you need to be a clinician who bills Medicare Part B for more than $90,000 in allowed charges, provides care for more than 200 Medicare patients, and provides more than 200 covered professional services per year. You also need to be one of the following clinician types:
